Hydroxyurea (HU) can cause rare leg ulcers in patients with myeloproliferative disorders. Stopping HU treatment effectively healed these painful ulcers in all reported cases.

Area of Science:

  • Dermatology
  • Hematology
  • Oncology

Background:

  • Hydroxyurea (HU) is a vital antineoplastic agent for myeloproliferative disorders.
  • Dermatological side effects of HU are common, but leg ulcers are rare.
  • Early recognition of HU-induced leg ulcers is crucial for timely intervention.

Observation:

  • Four cases of leg ulcers associated with chronic hydroxyurea therapy are presented.
  • Patients with chronic myeloid leukemia and polycythemia vera developed perimalleolar ulcers.
  • Ulcers varied in location (perimalleolar, lateral malleolar, medial malleolar) and duration.

Findings:

  • All four patients experienced painful leg ulcers linked to hydroxyurea use.
  • Ulcer healing was consistently observed upon discontinuation of hydroxyurea.

Related Experiment Videos

  • The severity of ulcers did not always correlate with HU dosage or duration.
  • Implications:

    • This report highlights the association between chronic hydroxyurea therapy and perimalleolar leg ulcers.
    • Increased physician awareness can lead to earlier diagnosis and effective management of these ulcers.
    • Promptly stopping hydroxyurea offers a successful treatment strategy for these debilitating ulcers.
