    Background:

    • Chenodeoxycholic acid (CDCA) is used to treat gallstones.

    Purpose of the Study:

    • To assess the potential toxic effects of chenodeoxycholic acid (CDCA) therapy on liver histology and function.
    • To evaluate changes in liver biopsies and serum liver function tests after 6-8 months of CDCA treatment in patients with gallbladder stones.

    Main Methods:

    • Liver biopsies were performed before and after 6-8 months of daily CDCA (750 mg) therapy in 6 patients with gallstones.
    • Histopathological analysis included light and electron microscopy.
    • Serum liver function tests (aspartate aminotransferase, alpha-glutamyl transpeptidase) were monitored monthly in a group of 20 patients.

    Main Results:

    • Minor fatty changes and lipofuscin increased post-therapy; no significant damage noted on light microscopy.
    • Electron microscopy revealed parallel changes in hepatocytes and patchy loss of microvilli in biliary epithelium.
    • A significant increase in sinusoidal lipocytes (Ito cells) was observed in all 6 biopsied patients; aspartate aminotransferase levels rose slightly but remained within normal limits.

    Conclusions:

    • CDCA therapy at 750 mg daily for gallstones is associated with an increase in sinusoidal lipocytes and minor hepatic changes.
    • While aspartate aminotransferase levels showed a slight, significant increase, they remained within the normal range, suggesting limited acute hepatotoxicity.
    • Further research is warranted to understand the long-term implications of CDCA-induced lipocyte changes.
