Background:

  • Spinal dural arteriovenous fistulas (SDAVFs) are rare vascular malformations.
  • Progressive myelopathy can be a presenting symptom of SDAVFs.

Observation:

  • A patient presented with progressive myelopathy.
  • MR imaging revealed extensive dilated spinal cord veins.
  • Angiography identified two distinct SDAVFs: one at T7 and another at T5.

Findings:

  • The T7 fistula exhibited descending venous drainage.
  • The T5 fistula demonstrated ascending venous drainage.
  • Both fistulas were confirmed as the cause of the patient's symptoms.

Implications:

  • Therapeutic embolization is an effective treatment for SDAVFs.
  • Early diagnosis and intervention can prevent irreversible neurological damage.
  • This case contributes to understanding the varied presentations and successful management of multiple SDAVFs.
