Area of Science:

  • Neurology
  • Ophthalmology
  • Vestibular System Disorders

Background:

  • Vestibular system disorders can cause visual disturbances.
  • Visual tilt illusions are uncommon symptoms associated with vestibular dysfunction.
  • The brainstem, particularly in Wallenberg's syndrome, is a common site of impairment.

Observation:

  • A 21-year-old man presented with sudden, intense rotational vertigo.
  • He experienced a dramatic clockwise visual world rotation of approximately 160 degrees.
  • The visual tilt resolved over days, accompanied by a headache; all diagnostic tests were normal.

Findings:

  • The case presents a unique instance of severe visual tilt.
  • Normal neurological and vascular imaging suggests a non-structural cause.
  • The symptom's resolution and associated headache point towards a transient neurological event.

Implications:

  • Visual tilt may indicate peripheral vestibular or central nervous system involvement.
  • This presentation could represent a vestibular epileptic seizure.
  • Further investigation into vestibular epilepsy is warranted for unusual visual disturbances.
