Background:

Congenital complete atrioventricular block (CCAVB) is a rare condition affecting fetal cardiac conduction.
The etiology of CCAVB can be idiopathic or associated with maternal autoimmune conditions.

Observation:

Two cases of CCAVB were identified in 23,400 deliveries over 3 years.
Fetal echocardiography showed no structural cardiac abnormalities in affected fetuses.
Both mothers and infants tested positive for anti-Ro (SS-A) antibodies.

Findings:

The ventricular rates in the affected infants were 31 and 80 beats/min.
One infant required pacemaker implantation due to severe bradycardia.
The presence of anti-Ro (SS-A) antibodies suggests a potential autoimmune link.
