Summary

Scleredema of Buschke, a rare skin condition, can present with unusual chronic papules and pseudovesicular lesions. This case highlights a rare koebnerization phenomenon in a long-standing scleredema patient following bacterial infection.

Background:

  • Scleredema of Buschke is a rare connective tissue disorder characterized by skin induration.

Observation:

  • A long-standing adult case of classical scleredema of Buschke was observed.
  • The patient developed chronic, clustered papules and pseudovesicular lesions.

Findings:

  • The new lesions exhibited koebnerization, appearing after a bacterial infection of the upper back.
  • This represents an unusual manifestation of scleredema, particularly the development of papular and pseudovesicular lesions with koebnerization.

Implications:

  • This case expands the known clinical spectrum of scleredema of Buschke.
  • It suggests that inflammatory or infectious triggers can induce distinct secondary skin reactions in patients with pre-existing scleredema.
