Open Angle Glaucoma: Treatment

In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. Background:

  • Age-related macular degeneration (AMD) is a leading cause of vision loss.
  • Choroidal neovascularization (CNV) is a common complication of AMD.
  • Photodynamic therapy (PDT) is a treatment for wet AMD.

Purpose of the Study:

  • To evaluate changes in electrooculography (EOG) following PDT.
  • To assess the impact of PDT on retinal pigment epithelium (RPE) function.

Main Methods:

  • Included 38 eyes of 38 patients with CNV secondary to AMD.
  • Administered standard PDT with verteporfin.
  • Performed serial EOG recordings pre-treatment, 1 week, and 1 month post-treatment.

Main Results:

  • Visual acuity improved in 17 eyes, remained stable in 16, and decreased in 5 eyes by 1 week post-PDT.
  • A statistically significant reduction in the EOG Arden ratio was observed 1 week after PDT, persisting at 1 month.
  • Three eyes experienced new hemorrhages and decreased visual acuity within the first week.

Conclusions:

  • PDT may lead to a reduction in the EOG Arden ratio.
  • This reduction suggests that PDT could potentially affect RPE function.
  • EOG serves as a potential indicator for monitoring RPE health post-PDT.
