Background:

Deep neck infections are common clinical scenarios.
Fish bones are sharp foreign bodies with the potential to cause pharyngeal or esophageal perforation.

Observation:

A patient presented with a deep neck abscess.
Initial diagnostic imaging, including radiography and esophagoscopy, did not reveal the foreign body.
Computed tomography (CT) successfully identified an intracorporeally migrated fish bone.

Findings:

A deep neck abscess was successfully treated by surgical exploration and retrieval of an intracorporeally migrated fish bone.
Computed tomography is crucial for diagnosing fish bone migration when initial assessments are inconclusive.
