Sinonasal fibrosarcoma (SFS) is a rare cancer presenting with nasal obstruction. This case highlights successful endoscopic removal and preservation of the orbit in a patient with SFS.

Area of Science:

  • Oncology
  • Otorhinolaryngology
  • Surgical Pathology

Background:

  • Sinonasal fibrosarcoma (SFS) is a rare malignant neoplasm.
  • SFS often presents with symptoms mimicking other sinonasal conditions, including nasal obstruction and epistaxis.
  • Accurate diagnosis relies on histopathology and immunohistochemistry.

Observation:

  • A 58-year-old male presented with an 8-month history of left proptosis, recurrent epistaxis, and nasal obstruction.
  • Nasal endoscopy revealed a left-sided nasal neoplasm.
  • CT and MRI demonstrated extensive tumor invasion into the left nasal fossa, ethmoid sinuses, and erosion of the medial orbital wall.

Findings:

  • Complete tumor resection was achieved via endoscopic sinus surgery, with orbital preservation.

Related Experiment Videos

  • Histopathologic examination confirmed the diagnosis of sinonasal fibrosarcoma.
  • Four years post-surgery, follow-up imaging and endoscopy showed no evidence of recurrence.
  • Implications:

    • Endoscopic sinus surgery is a viable and effective approach for complete resection of sinonasal fibrosarcoma.
    • Preservation of the orbit is achievable with this minimally invasive technique.
    • This case suggests favorable long-term outcomes following complete surgical removal of SFS.
