Background:

Conventional Heaney's avascular hepatic resection involves simultaneous exclusion of the abdominal aorta.
Normothermic total hepatic vascular exclusion in dogs informed a novel modification.

Observation:

The modified technique omits simultaneous abdominal aorta exclusion.
Total hepatic vascular exclusion time was appropriately prolonged.
Successfully applied in 3 patients with central liver tumors.

Findings:

The modified Heaney's technique is simpler and safer than the conventional method.
Demonstrated efficacy in resecting primary hepatocellular carcinoma and hepatic cavernous hemangioma.
Facilitated resection of tumors in segments VII and VIII, and porta hepatis.
