Summary
Wegener's granulomatosis (WG) is a rare cause of thyroid inflammation, presenting as a thyroid nodule. This case highlights the importance of considering WG in the differential diagnosis of inflammatory thyroid lesions.

Background:
- A 71-year-old patient presented with symptoms suggestive of hyperthyroidism, including significant weight loss and a suppressed thyroid-stimulating hormone (TSH) level.
- Imaging revealed a 4-cm left thyroid nodule, initially investigated for malignancy or common thyroiditis.
Observation:
- Despite extensive work-up, including fine needle and true cut biopsies, the thyroid nodule's nature remained inconclusive.
- Unexpectedly, nodular lung lesions were discovered, and positive cytoplasmic ANCA (antineutrophil cytoplasmic antibodies) titers were detected.
Findings:
- Surgical biopsies of both the thyroid and lung demonstrated extensive necrosis with granulomatous inflammation, characteristic of Wegener's granulomatosis (WG).
- This case represents the first documented instance of WG affecting the thyroid gland.
