Budesonide (BUD) effectively induces remission in most autoimmune hepatitis (AIH) patients. However, budesonide treatment failure and side effects were primarily noted in patients with liver cirrhosis.
Area of Science:
- Hepatology
- Immunology
- Gastroenterology
Background:
- Autoimmune hepatitis (AIH) is typically managed with prednisone and azathioprine.
- Complete histological remission rates with standard therapy are suboptimal, affecting approximately 65% of patients.
- Budesonide (BUD) has emerged as a potential alternative treatment for AIH.
Purpose of the Study:
- To evaluate the efficacy and safety of budesonide (BUD) in patients diagnosed with autoimmune hepatitis (AIH).
Main Methods:
- Eighteen patients with AIH received budesonide (BUD) at 3 mg thrice daily for a minimum of 24 weeks.
- The study included patients with co-existing primary biliary cirrhosis or primary sclerosing cholangitis, and some with advanced liver fibrosis or cirrhosis.
- Clinical, biochemical, and histological assessments were performed to monitor treatment response and safety.
Main Results:
- A high rate of clinical and biochemical remission (83%) was observed in patients treated with budesonide (BUD).
- First-line budesonide therapy led to remission in 70% of patients, while second-line therapy resulted in long-term remission for all patients.
- Histological remission was achieved in three patients, and clinically significant side effects were predominantly seen in patients with liver cirrhosis.
Conclusions:
- Budesonide (BUD) demonstrates significant efficacy in inducing remission for the majority of autoimmune hepatitis (AIH) patients.
- Treatment failure and adverse events associated with budesonide were mainly concentrated in patients presenting with liver cirrhosis.
