Summary

A woman experienced acute coronary syndrome due to disseminated intravascular coagulation (DIC) after an abortion complication. Treatment of the DIC resolved the cardiac issue, suggesting a resolved thrombus as the cause.

Area of Science:

  • Cardiology
  • Obstetrics & Gynecology
  • Hematology

Background:

  • Post-abortion complications can lead to rare but severe systemic issues.
  • Disseminated intravascular coagulation (DIC) is a life-threatening condition affecting blood clotting.
  • Acute coronary syndrome (ACS) involves reduced blood flow to the heart muscle.

Observation:

  • A 40-year-old woman presented with ST-elevation myocardial infarction after a dilatation and curettage for retained placental tissue.
  • The patient developed disseminated intravascular coagulation (DIC), complicating her cardiac event.
  • Coronary reperfusion was contraindicated due to the active coagulopathy.

Findings:

  • Symptomatic management of myocardial ischemia was initiated.
  • Successful treatment of the underlying DIC was achieved.

Related Experiment Videos

  • Coronary angiography post-DIC treatment showed no obstructive lesions or vasospasm, indicating a resolved thrombus as the likely cause of ischemia.
  • Implications:

    • This case highlights the critical link between obstetric complications, coagulopathies like DIC, and cardiac events.
    • It underscores the importance of managing the underlying cause of DIC before considering cardiac interventions.
    • The spontaneous lysis of a cardiac thrombus in the context of resolved DIC is a significant finding.
