Summary

A 62-year-old woman experienced spontaneous remission of unusual skin lesions. Histopathology revealed tuberculoid granulomas, suggesting a rare immune response.

Background:

  • A 62-year-old Japanese woman presented with multifocal skin lesions.
  • Lesions included atrophic scars and crusted, necrotic papules on extremities, chest, and back.

Observation:

  • Skin lesions exhibited spontaneous remission.
  • A tuberculin test showed a strongly positive reaction with induration and bulla.
  • Skin biopsy revealed tuberculoid granulomas with necrosis in dermal and adipose tissues.

Findings:

  • Bacterial cultures from sputum, urine, and stool were negative.
  • Chest X-ray was normal, ruling out active pulmonary tuberculosis.
  • Complete resolution of skin lesions occurred within one month post-biopsy without relapse.

Implications:

  • This case highlights a rare presentation of cutaneous granulomatous disease.
  • Spontaneous remission and resolution post-biopsy suggest an immune-mediated process.
  • Further investigation into the specific immune triggers for such granulomatous skin reactions is warranted.
