Background:

Autoimmune hepatitis (AIH) is a chronic liver disease of unknown etiology.
Uveitis is inflammation of the middle layer of the eye, which can be associated with systemic diseases.
Previous reports suggested an association between AIH and iridocyclitis, predating definitive hepatitis C virus (HCV) testing.

Observation:

A 38-year-old male presented with abnormal liver function tests and conjunctival hyperemia.
Initial symptoms six years prior, including fever and arthralgias, were misdiagnosed as acute rheumatic fever.
Ophthalmologic examination revealed anterior chamber inflammation and corneal stromal lesions.

Findings:

The patient exhibited positive antinuclear antibodies (ANA) and anti-smooth muscle antibodies (ASMA), with negative anti-LKM antibodies.
