Summary

Malignant phyllode tumor (PT) of the breast rarely metastasizes. This case highlights a unique instance of isolated duodenal metastasis from breast PT, successfully treated with pancreaticoduodenectomy.

Background:

  • Phyllode tumor (PT) is a rare breast neoplasm.
  • Malignant PT can metastasize distantly in 10-20% of cases, typically to lungs and bones.

Observation:

  • A 31-year-old woman presented with massive upper gastrointestinal bleeding.
  • Imaging revealed a large duodenal mass; she had a history of recurrent malignant PT treated with mastectomy 4 years prior.

Findings:

  • Pathologic examination confirmed the duodenal mass as a metastatic malignant PT, representing the first documented case of isolated duodenal metastasis from breast PT.
  • The metastasis likely occurred via hematogenous spread.

Implications:

  • This case expands the known metastatic sites for malignant PT.
