Summary

This study reports the first instance of fluorodeoxyglucose F-18 (FDG) uptake in a calyceal diverticulum, a rare kidney condition. The FDG-avid lesion was detected during a positron emission tomography (PET) scan for tumor screening.

Background:

  • Positron emission tomography (PET) scans using fluorodeoxyglucose F-18 (FDG) are standard for tumor screening.
  • FDG uptake in non-neoplastic renal lesions can sometimes mimic malignancy.

Observation:

  • A 46-year-old asymptomatic female presented with a large, FDG-avid lesion in the left renal pelvis on PET-CT.
  • The lesion demonstrated significant FDG uptake (SUVmax 10.7 at 1 hour, 3.9 at 3 hours).
  • Abdominal CT revealed a 3.7-cm cystic lesion in the left kidney, initially suspected as a complicated cyst.

Findings:

  • Histopathology confirmed the lesion as a calyceal diverticulum.
  • This represents the first documented case of significant FDG uptake in a calyceal diverticulum.

  • Calyceal diverticula should be considered in the differential diagnosis of FDG-avid renal lesions.
  • This finding may impact the interpretation of PET-CT scans in patients with renal abnormalities.
