Summary
Inner myometrial lacerations are a rare cause of postpartum hemorrhage. Prompt surgical repair during laparotomy can effectively control bleeding and prevent hysterectomy.
Area of Science:
- Obstetrics and Gynecology
- Surgical Management
- Hemorrhage Control
Background:
- Postpartum hemorrhage (PPH) is a leading cause of maternal morbidity and mortality.
- While common causes of PPH are well-documented, less frequent etiologies require consideration.
- Inner myometrial lacerations represent a challenging diagnostic and therapeutic entity in PPH.
Observation:
- A case of massive PPH following spontaneous vaginal delivery is presented.
- The hemorrhage was refractory to medical management and uterine packing.
- Laparotomy revealed a 4-cm inner myometrial laceration with active arterial bleeding.
Findings:
- Surgical repair of the inner myometrial laceration successfully controlled the PPH.
- This intervention avoided the need for hysterectomy.
- Diagnosis of inner myometrial laceration is critical when other PPH causes are excluded.
Implications:
- Inner myometrial lacerations should be included in the differential diagnosis for severe PPH.
- Timely surgical intervention, including laparotomy and hysterotomy for repair, is crucial.
- Effective management of inner myometrial lacerations can preserve fertility by preventing hysterectomy.
