Angiotensin-converting enzyme insertion/deletion polymorphism and systemic lupus erythematosus: a metaanalysis

This study found no link between the angiotensin-converting enzyme (ACE) I/D gene polymorphism and systemic lupus erythematosus (SLE) or lupus nephritis (LN). Further research is needed to understand the genetic factors contributing to these conditions.

Purpose of the Study:
- To investigate the association between the angiotensin-converting enzyme (ACE) insertion/deletion (I/D) polymorphism and susceptibility to SLE and LN.
- To analyze the ACE I/D polymorphism in different ethnic populations to identify potential ethnic-specific associations.
Main Methods:
- A comprehensive literature search was conducted using Medline and manual searches to identify relevant studies on ACE I/D polymorphism and SLE.
- A meta-analysis was performed on data from 13 comparison studies, including 1411 SLE patients and 1551 controls.
- Analyses included the DD genotype (recessive model), DD and DI genotypes (dominant model), and the D allele, examining overall and ethnic-specific associations, as well as associations with LN.
Main Results:
- The meta-analysis of 2962 subjects revealed no statistically significant association between the ACE I/D polymorphism and SLE across all ethnic groups.
- A non-significant trend towards association was observed for the DD genotype and D allele with SLE in Caucasian patients.
- No association was found between ACE I/D polymorphisms and the development or progression of lupus nephritis.
Conclusions:
- The ACE I/D polymorphism is not associated with an increased risk of developing systemic lupus erythematosus.
- The ACE I/D polymorphism does not appear to be a significant genetic risk factor for lupus nephritis.
- Current evidence suggests that the ACE I/D polymorphism does not play a major role in the pathogenesis of SLE and LN.
