Summary

Regular duplex ultrasound surveillance of pedal bypass grafts is crucial. Timely interventions like patch-angioplasty and balloon angioplasty can prevent graft thrombosis and ensure limb salvage in critical limb ischemia.

Area of Science:

  • Vascular Surgery
  • Interventional Cardiology

Background:

  • Pedal bypass grafts are vital for limb salvage in critical limb ischemia.
  • Excellent long-term outcomes are achievable but depend on vigilant patient monitoring.
  • Prompt identification and treatment of bypass stenosis are essential to prevent graft failure.

Observation:

  • A case of femoropedal bypass graft is presented.
  • Duplex ultrasound surveillance revealed critical stenoses at 18 and 36 months post-operatively.
  • These findings highlight the potential for recurrent stenosis in pedal bypasses.

Findings:

  • An 18-month stenosis was successfully treated with patch-angioplasty.
  • A 36-month stenosis was managed with percutaneous balloon angioplasty.
  • Both interventions effectively preserved the patency of the pedal bypass graft.

Related Experiment Videos

Implications:

  • This case underscores the importance of regular duplex follow-up for pedal bypass grafts.
  • Early detection and intervention for stenosis can maintain graft patency.
  • Such management strategies contribute to successful long-term limb salvage in patients with critical limb ischemia.
