This study reports a rare case of simultaneous lumbar disc herniation and spinal epidural hematoma in a teenager, successfully treated with surgery. Early diagnosis via MRI is crucial for effective management of this unusual cause of spinal stenosis.

Background:
- Lumbar disc herniation and spinal epidural hematomas (SEHs) are uncommon causes of secondary lumbar canal stenosis in adolescents.
- This report details a unique concomitant occurrence in a 16-year-old male presenting with L-5 radiculopathy.
Observation:
- MRI revealed a left posterolateral L4-5 disc prolapse with an extruded fragment and thecal compression.
- T2-weighted imaging showed a hyperintense area suggestive of the extruded disc fragment.
Findings:
- Surgical intervention successfully removed the encapsulated extradural hematoma and the extruded disc fragment.
- Biopsy confirmed the epidural collection as a hematoma.
- The patient experienced an excellent recovery, returning to normal activities within 6 months.
Implications:
- Spinal epidural hematomas can arise from minor trauma or disc extrusion, occurring at any age.
- Preoperative characterization with MRI is vital for differentiating SEH from disc herniation.
- Prompt surgical evacuation of concomitant SEH and disc herniation leads to favorable outcomes in adolescent spinal stenosis.
