Summary

Diffusion-weighted magnetic resonance imaging reveals unique serial changes in acute encephalitis or encephalopathy. These findings, including evolving lesions and eventual atrophy, are not seen with conventional imaging methods.

Background:

  • Diffusion-weighted magnetic resonance imaging (DW-MRI) is a sensitive tool for detecting cerebral lesions in neurological disorders.
  • Acute encephalitis and encephalopathy are serious conditions affecting the brain.

Observation:

  • Two pediatric patients with acute encephalitis/encephalopathy exhibited similar serial changes on DW-MRI.
  • Patient 1: Hemiconvulsion-hemiplegia-epilepsy syndrome. Patient 2: Acute encephalitis with exanthema subitum.

Findings:

  • DW-MRI showed high-intensity lesions in subcortical white matter during the acute phase.
  • Lesions progressed to involve the cortex, basal ganglia, or both.
  • In the convalescent phase, lesions resolved, and brain atrophy was observed.

  • Implications:

    • The observed DW-MRI changes may indicate specific pathogenic mechanisms in acute encephalopathy and encephalitis.
