Summary

Acute parkinsonism after hydrocephalus shunt revision resolved with levodopa. Symptoms did not return after stopping medication, suggesting a reversible cause related to hydrocephalus impacting the dopamine pathway.

Area of Science:

  • Neurology
  • Neurosurgery

Background:

  • Hydrocephalus is a condition characterized by excess cerebrospinal fluid in the brain.
  • Shunt revision surgery is performed to address complications or malfunctions in shunts used to treat hydrocephalus.

Observation:

  • A patient developed acute parkinsonian symptoms post-shunt revision for hydrocephalus.
  • Parkinsonism symptoms significantly improved with levodopa treatment.

Findings:

  • Levodopa therapy led to dramatic improvement in parkinsonism.
  • Parkinsonism did not recur even after levodopa discontinuation.

Implications:

  • The findings suggest hydrocephalus can cause reversible dysfunction in the nigrostriatal dopaminergic pathway.
  • This case highlights a potential link between hydrocephalus management and temporary parkinsonism.
