Summary
Juxta-facet cysts, intraspinal lesions, can mimic disc herniations. Surgical removal of an L4-5 synovial cyst relieved radicular symptoms and neurological deficits in a patient.

Background:
- Synovial and ganglion cysts, also termed juxta-facet cysts, are intraspinal lesions.
- These cysts can present symptoms similar to lumbar disc herniations, causing radicular pain.
Observation:
- A 75-year-old male experienced progressive left leg pain and foot numbness, with motor deficit in the extensor hallucis longus and L4-5 dermatome hypoesthesia.
- Magnetic resonance imaging identified an intraspinal cyst originating from the L4-5 facet joint.
Findings:
- Surgical intervention, including medial facetectomy and cyst excision, was performed.
- The patient experienced immediate pain relief following the procedure.
Implications:
- Surgical excision of juxta-facet cysts is an effective treatment for relieving radicular symptoms.
- Prompt surgical management can lead to rapid neurological recovery and improved patient outcomes.
