We present a case of Lactococcus garvieae endocarditis, a rare heart valve infection. This diagnosis was confirmed using genetic sequencing in a patient with prior bacteremia and heart failure symptoms.

Area of Science:

  • Infectious Diseases
  • Cardiology
  • Microbiology

Background:

  • Native-valve endocarditis is a serious infection of the heart valves.
  • Lactococcus garvieae is an emerging pathogen, rarely implicated in human endocarditis.
  • Enterococcus hirae bacteremia can present similarly to other serious infections.

Observation:

  • A patient presented with symptoms of congestive heart failure.
  • The patient had a history of Lactococcus garvieae bacteremia one month prior.
  • Enterococcus hirae bacteremia was detected during the current presentation.

Findings:

  • Definite Lactococcus garvieae native-valve endocarditis was diagnosed.
  • 16S rRNA gene sequencing confirmed the identity of both L. garvieae and Enterococcus hirae isolates.

Related Experiment Videos

  • Aortic valve vegetations were identified, confirming the site of infection.
  • Implications:

    • This case highlights the importance of considering L. garvieae in endocarditis, especially with prior bacteremia.
    • Accurate species identification via genetic sequencing is crucial for appropriate treatment.
    • Understanding the spectrum of Lactococcus infections is vital for clinical management.
