Two siblings developed fatal encephalitis after respiratory infections. This first report in relatives suggests genetic factors influence postinfectious encephalitis.

Area of Science:

  • Neurology
  • Immunology
  • Genetics

Background:

  • Postinfectious encephalitis is an inflammatory brain condition often triggered by infections.
  • While typically sporadic, the role of host genetics in susceptibility is not fully understood.

Observation:

  • A 33-year-old male and his 32-year-old sister presented with acute fatal encephalitis.
  • Both cases occurred following an upper respiratory tract infection of unknown cause, with symptom onset 2 years apart.

Findings:

  • Autopsy confirmed severe postinfectious encephalitis in both siblings.
  • This represents the first documented instance of postinfectious encephalitis in first-degree relatives.

Implications:

  • The familial occurrence strongly suggests that specific genetic factors contribute to the pathogenesis of postinfectious encephalitis.
