Summary

Brucellosis can cause difficult-to-diagnose spinal infections (spondylodiscitis), especially in older adults with existing back conditions. Early suspicion and serologic screening are key for timely diagnosis and treatment of this zoonotic disease.

Area of Science:

  • Infectious Diseases
  • Rheumatology
  • Radiology

Background:

  • Brucellosis is a prevalent zoonotic disease with osteoarticular involvement as a frequent complication.
  • Brucellar spondylodiscitis diagnosis can be challenging due to non-specific clinical presentations and overlapping conditions.

Observation:

  • An uncommon case of brucellar spondylodiscitis in an elderly male with diabetes and degenerative spinal disease is presented.
  • Magnetic resonance imaging (MRI) and a positive brucella-agglutination test aided in diagnosis.
  • Blood culture confirmed the diagnosis of brucellosis.

Findings:

  • The case highlights the diagnostic difficulties of brucellar spondylodiscitis, particularly in elderly patients with comorbidities.
  • Integrated diagnostic approach using serology and advanced imaging is crucial.

Related Experiment Videos

Implications:

  • Increased clinical suspicion for brucellar spondylitis is vital for prompt treatment, especially in endemic areas.
  • Wider use of serologic screening for brucellosis in patients with prolonged back pain is recommended.
  • Brucellar spondylodiscitis should be considered in the differential diagnosis of unexplained back pain in endemic regions.
