Summary

First-generation hormonal contraceptives do not cause focal nodular liver hyperplasia but can stimulate growth. Long-term use, especially of mestranol-containing pills, may promote tumor progression in benign liver neoplasias.

Area of Science:

  • Hepatology
  • Endocrinology
  • Oncology

Context:

  • First-generation hormonal contraceptives are associated with liver neoplasias.
  • Focal nodular hyperplasia (FNH) is a benign liver tumor.
  • Long-term oral contraceptive use is a known risk factor for liver adenomas.

Purpose:

  • To investigate the relationship between first-generation hormonal contraceptives and focal nodular liver hyperplasia.
  • To understand the pathogenetic mechanisms of contraceptive-induced liver tumor growth.
  • To analyze the characteristics and progression of FNH in relation to oral contraceptive use.

Summary:

  • First-generation hormonal contraceptives do not cause focal nodular liver hyperplasia (FNH) but can stimulate the growth of pre-existing lesions.
  • Prolonged intake of oral contraceptives, particularly those containing mestranol, is implicated in the tumorigenic progression of benign liver neoplasias.
  • FNH exhibits a high female prevalence (90-100%) and typically regresses after cessation of contraceptive use. This case involves 14 years of oral contraceptive use and an 8 x 6 x 4 cm FNH.

Impact:

  • Highlights the importance of considering oral contraceptive history in patients with FNH.
  • Informs clinical management regarding the risks associated with long-term hormonal contraceptive use.
  • Contributes to understanding the hormonal influences on benign liver tumor development and progression.
