Subclinical hypothyroidism significantly increases depression risk in older adults. Early thyroid screening is crucial for identifying and managing this risk in the elderly population.
Area of Science:
- Endocrinology
- Geriatrics
- Psychiatry
Background:
- Subclinical hypothyroidism, characterized by elevated serum thyrotropin (TSH) with normal free thyroxine (fT4), is common in the elderly.
- The association between subclinical hypothyroidism and depression in this age group requires further investigation.
Purpose of the Study:
- To investigate whether subclinical hypothyroidism is a risk factor for depression in individuals over 60 years old.
- To determine the prevalence of depression in elderly individuals with elevated TSH levels.
Main Methods:
- A cohort of 323 elderly individuals (≥60 years) were assessed for mood disturbances using the Structured Clinical Interview for DSM-IV.
- Participants were categorized based on serum TSH levels and depression diagnosis.
- Serum TSH, fT4, and thyroid antibodies were measured using sensitive assays and IRMA.
Main Results:
- Depression was diagnosed in 9.5% of individuals with elevated TSH (Group I) and 30.9% of depressed patients (Group II) had elevated TSH.
- Depression was significantly more frequent in subclinical hypothyroidism (49.7%) compared to overt hypothyroidism (16.8%) (p < 0.001).
- Subclinical hypothyroidism was associated with a more than fourfold increased risk of depression (OR = 4.886).
Conclusions:
- Subclinical hypothyroidism is a significant risk factor for depression in the elderly.
- Routine thyroid screening is recommended for elderly individuals to detect subclinical hypothyroidism and mitigate depression risk.
