Summary

Posterior shoulder instability is rare. This case highlights a patient with voluntary posterior subluxation who experienced traumatic instability, requiring surgery despite prior nonoperative treatment.

Background:

Posterior shoulder instability is uncommon, classified as traumatic or atraumatic, and voluntary or involuntary.
Voluntary posterior instability typically lacks a trauma history and often responds to physical therapy.

Observation:

A patient with voluntary, atraumatic posterior subluxation developed symptomatic traumatic posterior instability.
Nonoperative interventions, including rehabilitation, failed to restore preinjury function.

Findings:

The patient required operative treatment for a posterior labrum lesion.
This case presents a rare coexistence of voluntary, atraumatic, and traumatic posterior shoulder instability.
