Tumor Progression

Tumor progression is a phenomenon where the pre-formed tumor acquires successive mutations to become clinically more aggressive and malignant. In the 1950s, Foulds first described the stepwise progression of cancer cells through successive stages.
Colon cancer is one of the best-documented examples of tumor progression. Early mutation in the APC gene in colon cells causes a small growth on the colon wall called a polyp. With time, this polyp grows into a benign, pre-cancerous tumor. Further...

Background:

  • Accurate staging of lymph node metastasis is crucial for treatment planning and prognosis.
  • Understanding gene expression differences based on metastasis extent can reveal novel biomarkers.

Purpose of the Study:

  • To investigate gene expression profiles in human cervical tumors.
  • To correlate these profiles with the extent of lymph node metastases at diagnosis.
  • To identify potential molecular biomarkers associated with survival outcomes.

Main Methods:

  • Eight patients with invasive squamous cell carcinoma of the cervix underwent pretreatment whole-body fluorodeoxyglucose-positron emission tomography (FDG-PET) imaging.
  • Tumor tissue samples were analyzed using laser-capture microdissection and gene expression microarrays (Affymetrix Human U95A).
  • Supervised clustering analysis identified differentially expressed genes between patients with and without supraclavicular lymph node involvement.

Main Results:

  • FDG-PET imaging identified lymph node involvement in three patients and no involvement in five.
  • Gene expression analysis revealed 75 transcripts with at least threefold difference in expression.
  • Twelve transcripts showed statistically significant differential expression (P < 0.05), with five upregulated and seven downregulated.
  • Significant differences in overall and cause-specific survival were observed between the two patient groups (P = 0.006).

Conclusions:

  • This study identified candidate gene expression biomarkers associated with the extent of lymph node metastases in cervical cancer.
  • These biomarkers correlate with survival outcomes, suggesting their potential utility in predicting prognosis.
  • Further validation in larger cohorts is warranted to establish these findings.
