Purpose of the Study:

- To determine the impact of this multimodal approach on survival, local control, and metastatic disease.
Main Methods:
- A cohort of 67 patients with primary or recurrent retroperitoneal soft-tissue sarcoma treated between 1991 and 2004 were analyzed.
- All patients underwent maximal resection and IOERT (mean dose, 15 Gy).
- Forty-five patients received additional postoperative EBRT; 20 patients had prior irradiation.
Main Results:
- The 5-year actuarial overall survival was 64%, with 40% local control and 50% freedom from metastatic disease.
- Local control within the IOERT field was 72% at 5 years.
- Patients achieving R0 resection and completing both IOERT and EBRT had 80% 5- and 10-year overall survival and 100% 5- and 10-year local control, with acceptable morbidity (21% Grade II+ late complications).
Conclusions:
- Intraoperative electron-beam therapy (IOERT) in conjunction with maximal resection and radiotherapy provides excellent local control for retroperitoneal soft-tissue sarcomas.
- This combined modality treatment leads to favorable long-term survival in selected patients.
- The morbidity associated with this approach is acceptable.
