Summary

Occupational exposure to pepsin can cause allergic rhinoconjunctivitis. This case report details the first use of IgE-immunoblotting to confirm pepsin allergy in a slaughterhouse worker.

Area of Science:

  • Allergology
  • Occupational Medicine
  • Immunology

Background:

  • Occupational allergies can significantly impact worker health.
  • Pepsin has been previously linked to occupational asthma.
  • Diagnosing the causative agent is crucial for effective management.

Observation:

  • A slaughterhouse worker developed work-related rhinoconjunctivitis.
  • Symptoms were attributed to inhalation of pepsin.
  • Standard allergy tests (prick tests, conjunctival challenge) were positive for pepsin.

Findings:

  • Serum specific IgE levels for pepsin were elevated (5.58 kU/L).
  • SDS-PAGE immunoblotting identified a specific IgE-binding band at 43 kDa.
  • Symptoms resolved upon removal from pepsin exposure, confirming the diagnosis.

Implications:

  • This case highlights pepsin as a potential allergen causing rhinoconjunctivitis.
  • It demonstrates the utility of IgE-immunoblotting in diagnosing occupational allergies.
  • Further research into pepsin-induced occupational allergies is warranted.
