Botulinum toxin type A (BTX-A) injections reduced nerve growth factor (NGF) in bladder tissue for neurogenic detrusor overactivity patients. This treatment improved urodynamic parameters and decreased incontinence episodes.

Background:

  • Neurogenic detrusor overactivity (NDO) is a condition characterized by involuntary bladder contractions.
  • Visceral afferent nerve transmission plays a role in NDO pathophysiology, with nerve growth factor (NGF) implicated.

Purpose of the Study:

  • To investigate the effect of intravesical botulinum toxin type A (BTX-A) on visceral afferent nerve transmission in NDO patients.
  • To measure bladder tissue NGF levels before and after BTX-A treatment.
  • To correlate NGF levels with clinical and urodynamic outcomes.

Main Methods:

  • 23 NDO patients received intravesical BTX-A treatment.
  • Clinical evaluation and urodynamics (UDC threshold, maximum pressure, maximum cystometric capacity) were performed at baseline and 1, 3 months post-treatment.

  • Bladder biopsies were obtained for NGF level measurement using ELISA.
  • Main Results:

    • BTX-A treatment significantly decreased catheterization and incontinence episodes at 1 and 3 months.
    • Urodynamic parameters showed significant improvement: increased UDC threshold and maximum cystometric capacity, and decreased UDC maximum pressure.
    • Bladder tissue NGF levels were significantly reduced at 1 and 3 months post-treatment.

    Conclusions:

    • Intravesical BTX-A treatment leads to NGF deprivation in bladder tissue for at least 3 months.
    • Reduced acetylcholine release may decrease detrusor contractility and NGF production.
    • BTX-A might also affect other neurotransmitters modulating NGF production and release.
