Summary

Common variable immunodeficiency (CVID) patients can develop extranodal marginal zone lymphoma. Eradicating Helicobacter pylori infection is key for H. pylori-positive cases, while antibiotics may help H. pylori-negative cases.

Background:

  • Common variable immunodeficiency (CVID) is a primary immunodeficiency characterized by low immunoglobulin levels and impaired B-cell function.
  • Extranodal marginal zone lymphomas, previously known as MALT lymphomas, can arise in various extranodal sites.
  • The association between CVID, chronic inflammation, and lymphoproliferative disorders is increasingly recognized.

Observation:

  • Two CVID patients developed extranodal marginal zone lymphoma.
  • One patient had gastric lymphoma associated with H. pylori and pernicious anemia; lymphoma regressed after H. pylori eradication.
  • Another patient developed parotid gland lymphoma, which regressed with clarithromycin treatment despite being H. pylori-negative.
