Area of Science:

  • General Surgery
  • Minimally Invasive Surgery

Background:

  • The clinical benefit of laparoscopic appendicectomy compared to open appendicectomy is not definitively established.
  • Existing randomized studies have been analyzed for their methodological limitations.

Purpose of the Study:

  • To critically evaluate the clinical and statistical pitfalls in randomized studies comparing laparoscopic and open appendicectomy.
  • To provide guidance for appraising current literature and designing future research.

Main Methods:

  • A systematic analysis of 38 randomized studies was performed.
  • Evaluated clinical aspects included surgeon expertise, antibiotic protocols, complication definitions, and blinding.
  • Assessed statistical issues such as outcome definitions, sample size, statistical methods, and study comparability.

Main Results:

  • A significant number of studies exhibit both clinical and statistical deficiencies.
  • Key limitations identified include uncertainty in surgeon experience, lack of blinding, and the exploratory nature of many trials.
  • Preliminary findings highlight the need for methodological improvements.

Conclusions:

  • Despite advancements, the superiority of laparoscopic appendicectomy over open appendicectomy remains unproven.
  • There is a critical need for rigorously designed, well-defined randomized controlled trials to resolve this clinical question.
