Summary

Sarcoidosis can mimic cancer-associated retinopathy, causing visual loss and night blindness. Prompt steroid treatment improved vision and retinal findings in this case.

Post-Streptococcal Uveitis: a Rare Entity.

Area of Science:

Background:

  • Cancer-associated retinopathy (CAR) presents with visual loss, photophobia, and night blindness.
  • A middle-aged patient with a history of cancer developed these symptoms.

Observation:

  • A 63-year-old man with prior tongue cancer presented with progressive vision loss, photophobia, and night blindness.
  • Ocular examinations were unremarkable, but electroretinography showed diffuse cone and rod dysfunction.
  • ICG angiography revealed widespread hyperfluorescent spots, and chest CT identified focal lesions.

Findings:

  • Histological diagnosis of the lesions was sarcoidosis.
  • Systemic steroid therapy led to gradual visual improvement and resolution of ICG anomalies.
  • Vision recovered significantly, with normal visual fields restored.

Implications:

  • Sarcoidosis can manifest as autoimmune retinal dysfunction.
  • Photophobia and night blindness may be initial symptoms of sarcoidosis.
  • This case highlights the importance of considering sarcoidosis in patients with unexplained visual disturbances and a cancer history.
