Summary

Hypothyroidism can cause severe physiological impairment, leading to cardiogenic shock unresponsive to standard treatments. Prompt thyroid hormone replacement dramatically improved the patient's condition, highlighting the critical role of thyroid function in cardiovascular reserve.

Area of Science:

  • Cardiology
  • Endocrinology

Background:

  • Undiagnosed hypothyroidism can present with severe cardiovascular complications.
  • Atrioventricular nodal re-entrant tachycardia can precipitate acute heart failure in susceptible individuals.

Observation:

  • A patient with minimal coronary artery disease developed cardiogenic shock during an episode of atrioventricular nodal re-entrant tachycardia.
  • The patient's condition was refractory to multiple vasopressor agents.

Findings:

  • The patient's unresponsiveness to pressors was attributed to an undiagnosed hypothyroid state.
  • Thyroid supplementation led to a dramatic recovery, resolving hypotension and elevated filling pressures.

Implications:

  • This case underscores the critical importance of assessing thyroid function in patients with unexplained cardiogenic shock.

Related Experiment Videos
