Area of Science:

  • Emergency Medicine
  • Radiology
  • Health Services Research

Background:

  • Existing head computed tomography (CT) guidelines are often presentation-specific.
  • Concerns about over-utilization of head CT in emergency departments (EDs) led to the development of an integrated guideline at Western Hospital.

Purpose of the Study:

  • To evaluate adherence to a newly developed integrated guideline for ordering emergent head CT scans in ED patients.

Main Methods:

  • A retrospective medical record review was conducted for ED patients who underwent head CT between April 1 and June 17, 2004.
  • Clinical information was compared against the integrated guideline recommendations.
  • Descriptive statistics were used to analyze the data.

Main Results:

Related Experiment Videos

  • Out of 231 cases reviewed, 65 (28.1%) showed abnormal CT findings.
  • The integrated guideline was followed in 217 cases (93.9% adherence).
  • Non-adherence occurred in 14 cases (6.1%), with only one scan showing unclear clinical significance.

Conclusions:

  • High compliance suggests the head CT guideline is clinically relevant and well-supported by ED physicians.
  • The guideline may align with or influence current ED ordering practices for head CT.
