Granulomatous pyelitis is a rare kidney inflammation linked to urinary obstruction. This study details its features, distinguishing it from infections and aiding diagnosis.

Background:

Urinary obstruction can rarely cause granulomatous inflammation in the pyelocalyceal system, mimicking infections like tuberculosis.
Previous studies on this condition are limited, with only seven reported cases.

Observation:

A comprehensive study identified five cases of granulomatous pyelitis among 112 kidney specimens with urinary obstruction.
Patients had a history of urolithiasis or ureteral stenosis, with prior stent placement, and presented with unilateral urinary obstruction.

Findings:

The pyelocalyceal system showed dilatation and contained material consistent with calcified debris and Tamm-Horsfall protein.
Severe, non-infectious granulomatous inflammation was observed, limited to the pelvic wall, with negative cultures for fungi and mycobacteria.
