  • Laboratory tests revealed elevated digoxin plasma levels and ECG abnormalities.

Findings:

  • This case is the first reported instance of elevated digoxin levels with toxicity symptoms due to telithromycin co-administration.
  • The drug interaction, deemed probable, may involve P-glycoprotein inhibition by telithromycin, reducing digoxin elimination.

Implications:

  • Clinicians must monitor for digoxin toxicity when prescribing telithromycin concurrently.
