This case study reports the first instance of elevated liver enzymes caused by oral vancomycin. This adverse drug event resolved upon vancomycin discontinuation, highlighting a potential risk with this antibiotic.

- The hepatic enzyme elevations were reversible upon cessation of the antibiotic.

- Monitoring of liver function may be warranted in patients receiving prolonged or repeated courses of oral vancomycin, particularly those with inflammatory bowel disease.
