Background:

  • Therapeutic inhibition of gastric acid secretion, particularly with H2-receptor antagonists, has raised concerns about achlorhydria.
  • The risk of long-term hypergastrinaemia has gained attention following studies on rats treated with omeprazole, showing gastric carcinoid development.

Purpose of the Study:

  • To investigate the long-term effects of potent gastric acid inhibition on gastric mucosa and gastrin levels.
  • To assess the potential link between hypergastrinaemia and gastrointestinal tumors.

Main Methods:

  • Follow-up of patients with ranitidine-resistant peptic ulceration on long-term high-dose omeprazole therapy.
  • Serial gastric biopsy specimens were collected for up to 5 years.
  • Monitoring of plasma gastrin levels and ECL cell status.

  • Patients exhibited complete ulcer healing and moderate hypergastrinaemia.

Conclusions:

  • While potent acid inhibition can lead to hypergastrinaemia and ECL cell hyperplasia, it did not result in dysplasia or tumors in this patient cohort.
  • Excessive hypergastrinaemia from overdosage of gastric secretion inhibitors should be avoided.
  • Monitoring plasma gastrin levels is recommended for patients undergoing long-term treatment with potent acid-blocking drugs.
