Summary

Common genetic factors significantly influence both recurrent otitis media and recurrent tonsillitis. This shared genetic basis explains much of the comorbidity between these two common childhood conditions.

BMJ open·2026

Area of Science:

  • Otolaryngology
  • Genetics
  • Epidemiology

Background:

  • Recurrent otitis media and recurrent tonsillitis are common childhood conditions.
  • The etiological factors underlying their comorbidity are not fully understood.

Purpose of the Study:

  • To determine the relative contributions of genetic and environmental factors to the association between recurrent otitis media and recurrent tonsillitis.
  • To investigate shared genetic influences on these two conditions.

Main Methods:

  • Utilized self-report questionnaire data from a population-based cohort of 9479 Norwegian twins.
  • Employed structural equation modeling to analyze twin data and estimate genetic and environmental effects.
  • Assessed lifetime prevalence and calculated tetrachoric correlations between monozygotic and dizygotic twins.

Related Experiment Videos

Main Results:

  • Lifetime prevalence for recurrent tonsillitis was 11.7% and for recurrent otitis media was 11.2%.
  • Tetrachoric correlations were higher in monozygotic than dizygotic twins, indicating genetic influence.
  • A biometric model incorporating additive genetic effects and individual environmental effects, with non-additive genetic effects for tonsillitis, provided the best fit.
  • Identified a substantial overlap in genetic factors influencing susceptibility to both conditions, with no significant sex differences.

Conclusions:

  • Common genetic factors play a significant role in the comorbidity of recurrent otitis media and recurrent tonsillitis.
  • The findings highlight a shared genetic etiology underlying these frequently co-occurring conditions.
