Summary

Progressive enophthalmos developed 23 years after orbital floor fracture. This case suggests masticatory muscle movements may cause silent sinus syndrome via aspiration of orbital fat.

Observation:

  • A patient presented with progressive enophthalmos 23 years post-orbital floor fracture.
  • Imaging revealed orbital fat herniation into the pterygopalatine fossa and maxillary sinus wall collapse.

Findings:

  • The findings suggest a potential link between masticatory muscle activity and SSS.
  • Aspiration of orbital fat into the pterygopalatine fossa due to masticatory movements is hypothesized.

Implications:

  • This mechanism offers a novel explanation for some cases of SSS.
