Background:

  • Assessing the impact of moderate versus deep intraoperative hypothermia on postoperative morbidity in patients undergoing arterial switch operation (ASO).
  • Evaluating outcomes in 71 newborns who underwent ASO, comparing moderate hypothermia (24-30°C) with deep hypothermia (16-22°C).

Purpose of the Study:

  • To determine if moderate intraoperative hypothermia is a safe and effective alternative to deep hypothermia for ASO.
  • To compare postoperative morbidity between moderate and deep hypothermia groups in ASO patients.

Main Methods:

  • Retrospective analysis of 71 newborns undergoing ASO, divided into moderate hypothermia (n=21) and deep hypothermia (n=50) groups.
  • Comparison of intraoperative temperatures, cross-clamping times, ultrafiltration volumes, and postoperative outcomes including mortality, chest closure, and extubation times.

Main Results:

  • Intraoperative temperatures averaged 25°C (moderate) and 19°C (deep).
  • No significant differences in cross-clamping time or ultrafiltration volume between groups.
  • One death in the deep hypothermia group due to myocardial ischemia; 1.4% in-hospital mortality overall. Moderate hypothermia group had fewer secondary chest closures (33% vs 54%) and faster extubation (7.4 vs 6 days).

Conclusions:

  • Arterial switch operation (ASO) can be safely performed using moderate hypothermia.
  • Moderate hypothermia demonstrates comparable safety and potentially better outcomes than deep hypothermia for ASO, even in complex cases.
