Background:

Vasovagal syncope is a common cause of fainting, often triggered by specific stimuli.
Dental procedures can precipitate vasovagal episodes due to patient anxiety, pain, and environmental factors.
Malignant vasovagal syncope represents an extreme form of this autonomic response.

Observation:

A 21-year-old male experienced recurrent loss of consciousness during dental appointments.
Diagnostic workup, including head-up tilt testing, revealed prolonged asystole and confirmed malignant vasovagal syncope.
Dental treatment was repeatedly interrupted due to these syncopal episodes.

Findings:

Malignant vasovagal syncope can manifest with severe autonomic dysfunction, including asystole.
The patient's condition was successfully managed with pharmacological therapy (disopyramide) and a dual-chamber pacemaker.
Effective treatment allowed for the eventual completion of necessary dental procedures.

Implications:

Dentists should be aware of the potential for severe vasovagal syncope in patients undergoing treatment.
Prompt cardiac evaluation is essential for patients presenting with unexplained syncope, particularly in a dental setting.
Integrated management involving cardiology and dentistry can improve patient outcomes and enable necessary medical care.
