Background:

  • Voluntary nystagmus, an unusual eye movement, has not been extensively studied for its pathological significance or familial occurrence.
  • Understanding the genetic and physiological underpinnings of voluntary nystagmus is crucial for diagnosing and managing related visual disturbances.

Observation:

  • Two families were evaluated using advanced techniques including video, search-coil, and electronystagmography (ENG).
  • Members across three generations in one family and a child in another demonstrated the ability to generate horizontal voluntary nystagmus.
  • Specific parameters like duration, amplitude, and frequency of the nystagmus varied between individuals and families.

Findings:

  • The nystagmus in the first family showed consistent characteristics (2-5s duration, 1-4° amplitude, ~15 cycles/sec frequency).
  • In the second family, a patient with esotropia and hyperopia produced nystagmus (5° amplitude, 10 cycles/sec, up to 20s duration) that also occurred involuntarily during reading, causing oscillopsia.
  • Corrective lenses (+0.75 diopters) eliminated the involuntary nystagmus during near fixation in this patient.

Implications:

  • The parameters of voluntary nystagmus seem to be inherited and family-specific.
  • Prescribing plus-lenses can be an effective strategy to mitigate involuntary nystagmus by reducing the convergence impulse.
  • Further research into the genetic and neurological basis of voluntary nystagmus is warranted.
