Interventricular septum dissection is a rare complication of acute myocardial infarction. This case highlights a successful non-surgical approach with close echocardiographic monitoring for managing this condition.

Background:

  • Interventricular septum (IVS) dissection is an uncommon complication of acute myocardial infarction (AMI).
  • Prompt diagnosis and appropriate management are crucial for patient outcomes.

Observation:

  • A case of unruptured IVS dissection diagnosed 16 days after two acute myocardial infarction episodes.
  • Transthoracic echocardiography revealed an echo-free space within the IVS, extending 30 mm from apex to mid-portion.
  • No discontinuity of the left ventricular wall was observed.

Findings:

  • The patient remained clinically stable with no flow within the dissection.
  • Favorable evolution was noted during hospitalization.
  • Contrast echocardiography showed reduced dissection area and increased echogenicity with organized thrombi.

  • Implications:

    • A non-surgical management strategy can be considered for interventricular septum hemorrhagic dissection.
