Area of Science:

  • Hematology
  • Oncology
  • Gastroenterology

Background:

  • Acute myeloid leukemia (AML) is a heterogeneous group of hematologic malignancies.
  • Obstructive jaundice is typically associated with biliary or pancreatic pathology.
  • Leukemic infiltration of the biliary system is an uncommon presentation.

Observation:

  • A 32-year-old man presented with symptoms of obstructive jaundice.
  • Imaging demonstrated biliary duct dilation and a urinary bladder mass.
  • The patient was diagnosed with the eosinophilic variant of AML M4 subtype.

Findings:

  • The patient's AML manifested with significant biliary obstruction.
  • Leukemic involvement of the urinary bladder was also noted.
  • The eosinophilic variant of AML M4 subtype was identified.

Implications:

  • This case highlights the diverse and unusual presentations of acute myeloid leukemia.
  • Early recognition of non-hematologic symptoms is crucial for timely diagnosis.
  • Aggressive disease course can preclude timely therapeutic intervention in AML.
